Wellness Workshops is a new health education series, held monthly. This session: The Transition Toolkit Preparing for Life's Care Transitions Whether returning home from hospital, managing a new diagnosis, or planning for future care needs, transitions can be challenging. This session offers practical guidance on care planning, home safety, community resources, and aging in place, helping individuals and families navigate change with confidence.
